Child and Teen Checkups (C&TC)

Screen for Health

Complete and regular screenings are the basis of the C&TC program. These exams are given on a set schedule to assure that health problems are diagnosed and treated early, before they become more complex and treatment more costly.

Some health plans offer rewards for staying up-to-date on well child exams, dental exams, and vaccines. Learn more about current Health Plan Rewards.

Eligibility

Children and teens, birth through age 20, covered by a Minnesota Health Care Program (Medical Assistance or Minnesota Care) are auto-enrolled in the Child and Teen Checkups program. For more information about the program, please call/text 763-335-0280.

Checkups Include:

  • Information on:
    • Physical health
    • Social and emotional health
    • Age appropriate growth & development
    • Parental guidance
    • Resources and services available
  • A complete physical exam, including:
    • Height, weight, body mass index (BMI)
    • Hearing and vision exam
    • Immunizations
    • Lab testing for lead, hemoglobin, and cholesterol
    • Dental fluoride application
  1. Birth-18 months
  2. 2-6 years
  3. 7-20 years

  • Between birth and 1 month
  • 2 months
  • 4 months
  • 6 months
  • 9 months
  • 12 months
  • 15 months
  • 18 months

The Child and Teen Checkups program provides FREE medical, dental, and vision checkups to Wright County residents who are age 20 or younger and enrolled in Medical Assistance.  Contact your health care plan to set up an appointment with a participating provider today!


Transportation is included in your Medical Assistance plan.  If you need help getting to a medical appointment, please contact your health plan.
