Child Care Assistance Program Application

To apply for the Child Care Assistance Program (CCAP), complete the forms in the section below and sign the application.  All requested verifications must accompany your application.  Return your completed application to Wright County Human Services, 1004 Commercial Drive, Buffalo, MN 55313-1736.

Wright County Human Services may require up to 30 days to process your application.  The results of your application will be mailed to you.
