Child and Teen Checkups (C&TC)

Screen for Health

Complete and regular screenings are the basis of the C&TC program. These exams are given on a set schedule to assure that health problems are diagnosed and treated early, before they become more complex and treatment more costly.

Some health plans offer rewards for staying up-to-date on well child exams, vaccines, and lab tests. Learn more about current health plan rewards


Children and teens, from birth to age 21, covered by a Minnesota Health Care Program (Medical Assistance or Minnesota Care) are auto-enrolled in the Child and Teen Checkup program. For more information about the program, please call 763-682-8921.

Checkups Include:

  • Information on:
    • Good physical health
    • Social and emotional health
    • Age appropriate growth & developmen
    • Parental guidanc
    • Resources and services available
  • A complete physical exam, including:
    • Hearing & vision exam
    • Height, weight, body mass index (BMI)
    • Immunizations
    • Lab testing for lead, hemoglobin, and cholestero
    • Dental fluoride application
