Guardianship / Conservatorship Request for Proposals
Wright County Health & Human Services has put forth a Request for Proposals seeking qualified applicants to become contracted guardians and/or conservators with our Agency. Beginning January 1, 2020, Wright County will be exclusively working with guardians and conservators who either have a contract in place with our Agency or who are only planning to receive 10% of a ward’s Social Security benefits as reimbursement for services. Providers planning to receive 10% of a ward’s Social Security benefits only will be able to continue doing so without responding to this RFP and contracting with the county.
Please review the Request for Proposals document below for detailed and specific information. The deadline to submit proposals is June 28, 2019 at 4:00pm. We are offering a two week period for questions. The final day to submit questions will be May 17, 2019 at 4:30pm. Please direct questions to Jill Pooler, firstname.lastname@example.org. Only emailed questions will be accepted. All answers to submitted questions will be posted through the link identified below. We look forward to reviewing your proposals.
